Southwestern Medical Center’s department of Patient Registration assists with a variety of check-in services including pre-authorization, admittance and scheduling.
Prior to admittance, we will submit and verify the patient’s current insurance information. If there are any difficulties with insurance approval or authorizations, we will notify patients prior to any medical procedures. For questions or concerns regarding pre-authorization please call 580-531-4735.
The following documentation is required when registering as a patient at Southwestern Medical Center.
- Current insurance card(s)
- Photo I.D.
- Social security card
- Advanced directive/Living Will (if applicable)
Patient Bill Pay
All co-pays and patient fees are due at the time of service.
For your convenience you may now pay your bill online. Have your account information ready and click the link below.
For patients who do not have medical insurance, do not qualify for government assistance, or who are underinsured, we offer a financial assistance program. Patients can determine eligibility for the financial assistance program by applying through the hospital business office. Financial assistance applications are due no later than 10 days post-discharge. For more information or for eligibility determination and application requests, call 866-656-9768.
Advance Directives and Living Wills
Southwestern Medical Center supports patient’s rights to make their own medical decisions. We encourage anyone receiving treatment to provide his or her living will, durable power of attorney, or health care proxy information prior to receiving care. Patients can provide a signed, witnessed, and dated copy to their medical provider to include in their medical charts.
